It’s less of a deliberate, top-down revolution and more of a grassroots, slightly chaotic scramble. And frankly, it’s time we talked about it.
A revealing new survey has just lifted the curtain on this quiet insurgency. It turns out that one in four General Practitioners in the UK are now using generative AI tools like ChatGPT in their day-to-day work. This isn’t just a handful of tech-savvy early adopters; it’s a significant chunk of the primary care workforce looking for an edge, a shortcut, or simply a way to keep their heads above water. The real story, however, isn’t that they’re using it, but how they’re using it—and the almost complete absence of any safety net.

The Shadow AI in the Surgery

Let’s break down the numbers, because they paint a stark picture. According to the study, published by researchers from institutions including Uppsala University and Harvard Medical School, the use of AI among GPs is surging. They’re turning to these tools for a variety of tasks, and while some are perfectly sensible, others should set alarm bells ringing.
About 35% are using it for the grunt work of documentation—summarising notes, drafting referral letters, and generally tackling the administrative beast that consumes so much of a doctor’s day. Fine. But look closer. A staggering 27% are using AI for help with differential diagnoses, essentially asking a chatbot, “Here are the symptoms, what could it be?” Another 24% are using it to help formulate treatment and referral plans.
This is where we move from a helpful administrative assistant to a risky, unregulated medical advisor. The pressures on GPs are immense, we all know that. Appointment slots are shrinking while patient complexity grows. In that context, reaching for a tool that promises instant answers is entirely understandable. It’s also deeply concerning.

Here is the most jaw-dropping statistic of all, as reported by Sky News: 95% of the GPs using these AI tools have had precisely zero formal training on them. Let that sink in for a moment. Ninety-five per cent.

Clinical trust factors are not built on promises of efficiency; they are painstakingly constructed through years of rigorous training, peer review, accountability, and demonstrable evidence of safety and efficacy. Patients trust their GP not just because they are knowledgeable, but because they operate within a system of checks and balances.
Where does a commercial chatbot fit into this? When an AI model suggests a wrong diagnosis or a harmful treatment, who is responsible?
– The GP who typed in the prompt?
– The hospital trust that tacitly allows its overworked staff to find their own solutions?
– The tech company in Silicon Valley that built the model but slaps a “for entertainment purposes only” disclaimer on it?
This ambiguity is toxic to trust. Approved, Vetted Tools: The NHS needs to move faster to test, approve, and provide access to medical-grade AI tools that have been specifically designed and validated for clinical use, not just adapt commercial products. This creates a safe, walled garden instead of the current free-for-all.
Clear Ethical Frameworks: We urgently need national guidelines on accountability, data privacy, and patient consent for the use of AI in diagnostics and treatment planning.
Without these measures, we are simply waiting for a major incident—a high-profile, AI-assisted medical error that will inevitably cause a public backlash and could set back genuine healthcare AI adoption by a decade.

The current state of affairs is untenable. We have a workforce that is simultaneously brilliant and unsupported, adopting powerful technology without the knowledge or framework to use it safely. It’s time to transform this shadow operation into a sanctioned, structured, and safe component of 21st-century medicine.
